As we implement the Patient Protection and Affordable Care Act (ACA) across our integrated Ministry nationwide, Ascension hospitals have each conducted a comprehensive Community Health Needs Assessment to identify and understand the health needs of our local communities beyond the walls of our hospitals.
Hospitals are required to provide and post publicly their CHNA reports every three years. Reports include the hospital’s assessment of the needs of their communities, as well as implementation strategies to address the identified needs.
Hospitals are required to take into account input from persons who represent the broad interest of the community served by the hospital facility.
The CHNA community-driven approach supports the hospital in planning and carrying out programs that will assist with handling the health needs of the underserved and vulnerable populations within their communities.
